Employer Agreement
This documents the roles and responsibilities of the student, the employer, the school, and the student's parent or guardian, and contains space for all of these parties' signatures. In order to satisfy the work experience requirements of the ProStart program, a signed copy of this agreement must be on file at the school, and student must adhere to the agreement throughout their ProStart work experience(s).
Exam Administration Handbook
The procedures for requesting, distributing and returning exams are outlined in this handbook that is updated each school year.

The information that needs to accompany your completed exams. This envelope is provided when you order your exams.

Use this to help recruit students for the ProStart program or to award internships.
Student Letter of Recommendation (sample)
A rough sketch of a letter from a student's mentor or worksite employer to future employers. It should be fleshed out with information specific to each student.
Student Survey (sample)
This form asks students to describe and rate their experience after each semester of working and studying the ProStart curriculum. A great way to get valuable feedback from the real beneficiaries of the program.

Included in the activity book, this checklist can be utilized when working with industry and students.
Student Workplace Validation Form
Used by the state restaurant association ProStart program coordinator, this form evaluates students' work experiences according to the requirements outlined by the National Restaurant Association Educational Foundation (NRAEF). Students must satisfy these requirements in order to be eligible for the ProStart National Certificate of Acheivement. The completed form must be signed by the state restaurant association ProStart program coordinator and submitted to the NRAEF in order to release the students certicate.
